Resident Rooms  

Overall, how are the rooms? Think about things like personal belongings, storage space, furniture, availability of electric beds, private bathrooms, windows, phone, television, choice of roommates, quiet areas, etc..

Living Spaces  

How do you like the living spaces overall? Is the facility free from overwhelming unpleasant odors? Is it clean and well kept? Is the decor pleasing and warm? Is the temperature comfortable? Does it have good lighting? Are the noise levels in the common areas comfortable? Are furnishings sturdy, comfortable, and attractive?

Resident Appearance  

Are the residents clean, appropriately dressed for the season or time of day, and well-groomed?

Staff  

Is the relationship between the staff and the residents warm, polite, and respectful? Does the staff wear name tags? Does Staff personnel knock on the door before entering a resident's room and refer to residents by name? Is there a full-time Registered Nurse (RN) available at all times? Are Social Workers and Doctor available?

Food  

How's the grub? Hopefully delicious! Do residents have a choice of food items at each meal? Are nutritious snacks are available upon request? Is the kitchen clean and orderly? Does a registered dietician, or professional service, plan the meals?

Activities  

Does the facility allow you to take part in a variety of activities? Are there outdoor areas for resident use? Are religious services or transportation to churches offered? Are there special outings? Does the facility have an active volunteer program?

Safety & Care  

Are fire drills practiced? Do residents get preventive care (like a yearly flu shot) to help keep them healthy? Can residents still see their regular doctors? Is there an arrangement with a nearby hospital for emergencies? Is in-house physical therapy provided? Is the facility vigilant to correct deficiencies (failure to meet Federal or state requirements) on its last state inspection report?
