Here’s What to Know About 2025’s Change to the Medicare Part D Donut Hole
Since the 2025 Part D plans became available on October 1 at Medicare.gov, the Toni Says Medicare team has reviewed the significant changes to Part D premiums and prescription drug costs. Melodye, I have great news for you: Effective January 1, the “Donut Hole” is going away!
You were right about the major change to the 2025 Medicare Part D plan, which aims to make prescription drugs more affordable for Americans on Medicare. With the elimination of Medicare’s famous “Donut Hole,” your maximum out-of-pocket cost for Part D prescriptions in 2025 will be $2,000. This will bring much-needed relief to many on Medicare!

Here’s a breakdown of the 2025 Medicare Part D costs:
- Initial Deductible: $590
- Initial Coverage Stage: During the five drug-tier stages of Initial Coverage, your Part D plan pays its share of your drug costs, and you pay your share until reaching the $2,000 out-of-pocket maximum. After that, you’ll move to Catastrophic Coverage.
The 5 Drug Tiers
Medicare Part D plans often categorize prescription drugs into five tiers. Each tier has a different copayment or coinsurance amount that you’ll pay. The specific costs can vary depending on your plan. Here’s a breakdown of the five drug tiers:
- Generic Drugs – Lowest-cost. These drugs typically have the lowest copayments or coinsurance amounts.
- Preferred Generic Drugs – These are generic drugs that your plan considers preferred. They might have even lower costs than standard generic drugs.
- Preferred Brand Drugs – These are brand-name drugs that your plan considers preferred. They generally have higher copayments or coinsurance amounts than generic drugs but lower than non-preferred brand drugs.
- Non-Preferred Drugs – These are brand-name drugs that your plan doesn’t consider preferred. They typically have the highest copayments or coinsurance amounts.
- Specialty Drugs – These are high-cost medications used to treat complex conditions. They often have unique coverage rules and may require prior authorization.
Drug tier copays vary based on the Part D or Medicare Advantage Plan you select. Also, the specific drugs in each tier can change from year to year, so you should review your Part D plan’s formulary (the list of covered drugs) annually.
Reason to Celebrate: Effective January 1, 2025, Medicare’s Donut Hole/Coverage Gap will no longer exist.

Catastrophic Coverage
In 2024, to reach catastrophic coverage, your total out-of-pocket costs for covered drugs had to reach $8,000. Once you reached catastrophic coverage, Medicare would pay 80% of the remaining drug costs, your Part D plan would pay 20%, and you would have no copay (0%) for covered medications for the rest of the year.
In 2025, since the “Donut Hole” will no longer exist, catastrophic coverage kicks in after you’ve paid $2,000 out-of-pocket.
This only applies to covered medications in your plan’s formulary.
Medicare Prescription Payment Plan Starts in 2025

This is a new option that allows you to spread your out-of-pocket drug costs throughout the year in the form of monthly payments, instead of paying a large sum at the pharmacy. It’s a separate program from catastrophic coverage and doesn’t affect the out-of-pocket maximum.

- Visit Medicare.gov to explore 2025 Medicare Part D and Medicare Advantage plans. The site offers tools to help narrow your search for new plans.
- Discuss with your primary care and specialty doctors about switching brand-name drugs to generics (you can even get samples from your doctor).
- Explore various prescription drug programs like GoodRx, Single Care, or Amazon for more affordable options. Retailers like Walmart, Kroger, HEB, and Costco also provide discount plans.
Remember, your prescription drug needs can change yearly. Review your options carefully, as what you don’t know can impact you!
